Here's a quick overview of what you'll be doing: :
    • Case manager adjacent role involving scheduling caregiver staff for hours of service on new clients, and open hours/shifts on existing cases to assure consistency in caregiver scheduling.
    • Meet with client, and family to accurately complete all admission paperwork.
    • Assure the client, and family understand the home care plan, and their responsibilities
    • Schedule, conduct, and document home visits to collect feedback from clients regarding caregiver performance of duties, and problem solve as required
    • Develop the plan of care in accordance with the client needs/wishes, and update as warranted.
    • Help maintain all agency marketing, client materials, and office supplies.
    • Help out in handling after hours on-call rotation with the other team members

    • At least one year of professional office experience
    • High school diploma required, and an associate's degree is preferred
