Job Requirements
Client Support Worker (CSW)
- Valid Drivers License
- Experience working with the Mental Health population environment (Preferred)
- Fingerprint Based Background Check of the Georgia and Federal Criminal History Record
- Drug Screen
- Motor Vehicle Report (MVR) for seven years
- High school or equivalent (Required)
- High School Diploma or GED
Job Description
Summary:
Under direct supervision, assists with daily living activities at the person’s home, in a daytime non- residential facility or a residential facility. Provides training of basic personal and social skills, simple academics, or work skills to consumers/students in a state school, hospital, juvenile detention facility, residence, group home, workshop, or supported employment environment.
Essential Duties and Responsibilities:
- Observation and documentation self-administration of medication - documenting any concerns
- Complete a medication count
- Assist with cooking meals
- Complete a daily bedroom and bathroom check providing directions as needed to ensure cleanliness; Ensure individuals are completing chores as assigned
- Treating each individual with respect, dignity and listening in a professional manner daily
- Facilitating groups to address treatment plan
- Completing clear and legible group notes and shift notes to reflect individual’s progress/regress in reaching treatment plans goals
- Ensure individuals attend doctor's appointments as assigned by Lead House Parent - documenting all concerns and uploading all discharge paperwork
- Interacting with each individual hourly outside of the staff office documenting any concerns/regressions and attending community activities with individuals as assigned
- Participate in monthly staff meetings and individual staffing
- Maintain compliance with all company policies and procedures
- Perform related duties as assigned by supervisor
