Clinical Pharmacist

The Clinical Pharmacist is an integral member of the interdisciplinary team providing telehealth services to Managed Long Term Support Services (MLTSS) patients in the CareBridge 24/7 Member Support Program. The Clinical Pharmacist provides direct-to-patient interventions that optimize the use of medication, improve medication adherence and promote improvement in health outcomes. This role also provides expertise to other members of the team in the management of patient health concerns.

Responsibilities:

• Respond to referrals from other members of the CareBridge team in a timely manner
• Interact with patient post-discharge from hospitalization and complete medication reconciliation
• Review medication reconciliations based on identified criteria
• Offer recommendations to the CareBridge provider and/or community Primary Care Providers and Specialists as appropriate, i.e. compliance with treatment guidelines for evidence-based medicine, simplify medication regimen for improved adherence, formulary alternatives, reduction of high-risk medications, evaluation of high-cost medications, etc.
• Document appropriately in accordance with CareBridge standards
• Participate in case conferences for complex patients
• Drives quality care and HEDIS measure closures
• Complete prior authorizations and refill requests on behalf of providers per company policy
• Willingness to obtain and maintain multiple state licenses as needed
• Perform other duties as assigned

Qualifications:

• Pharm D. degree from an accredited school of pharmacy
• Unrestricted license in state(s) as requested
• 5+ years working with high-risk adult chronic disease patients and/or seniors
• Experience working with an interdisciplinary team
• Experience with electronic medical records
• Medicare and Medicaid experience preferred
• Works and communicates well in a team environment
• Maintains a patient-centered focus
• Excellent communication skills
