Job Description

The Community Advocate/Case Manager will work at each of WestCOP’s Community

Action Program (CAPs), under the direction of that Program’s Area Director to

disseminate vital services and program information to the undeserved population

of the assigned area. CAs will work under the direction of the Area Director to

provide advocacy, information and referral and direct services and assistance to

the undeserved population of the assigned area.

Responsibilities

  • Identify and gain expertise about the services that are provided by the

CAP.

  • Expand and enhance services already provided by the CAP. Work with

the Director to identify those services that are in need of development.

Create a working plan to shore up and strengthen these service areas.

  • Work with the Director to develop new services, as needed, to serve

additional at risk individuals and families.

  • Address the immediate needs of high risk populations (i.e. newly arrived

residents, physically/mentally challenged, elderly, etc.) through referrals,

service advocacy, establishment of support groups, and, when needed,

direct case management.

  • Take full advantage of opportunities to increase the level of services,

including, but not limited to, development, referral and advocacy training

provided by WestCOP.

  • Provide outreach to additional at risk individuals and families.
  • Along with the Director, develop an intensive case management plan for a

minimum of 10 families.

  • Responsible for developing program goals, and evaluating the attainment

of these goals.

Job Description

Community Advocate

  • Responsible for local Center operations, as assigned, which may include

filling in at reception and phones and maintaining the aesthetics in the

office.

  • Responsible for maintenance of records recording to satisfy requirements

under ARRA.

  • Collection, collating, and inputting client data.
  • Conduct in-person and phone interviews with residents who contact the

CAP to determine the nature of the issue they have.

  • Make referrals to appropriate WestCOP program, government, or non-

governmental agency best able to assist client.

  • Follow up with clients to ensure issues are being addressed, and

intervene if necessary with referral agency to obtain services.

  • With approval from Director, establish and maintain contact with

community organizations to assure cooperation with the CAP to

accomplish goals and objectives.

  • With approval from Director, perform public relations on behalf of the CAP

and WestCOP.

  • Cooperate with other CAs, Directors and Central staff to accomplish

general goals of WestCOP.

Internal: Director, Other CAs and Directors, Supervisor of ARRA funds

External: Local School districts, Local Department of Social Services Office,

Local Unemployment Office, Local Health Office, other entities as

Director indicates

Minimum Requirements

  • BA or BS degree from an accredited, 4 year college or university
  • At least 3 years in Human Services or related field
  • Experience with at risk populations
  • Experience handling sensitive personal information with discretion
  • Bi lingual Spanish/English desired
  • Proficiency in Microsoft Office programs required

Job Type: Full-time

Pay: $21.97 - $24.72 per hour
