Community Health Specialist

BACKGROUND:

Urban Strategies is a 501(c) 3 corporation founded in 1978. The mission of Urban Strategies is to empower residents in distressed urban core neighborhoods to lead healthy, prosperous lives in thriving, self-sustaining communities. Urban Strategies plans and implements Human Capital building strategies designed to end the physical, social and economic isolation of obsolete and distressed communities by 1) creating an attractive mixed income community and 2) supporting former low-income residents in progressing toward self-sufficiency. Urban Strategies uses innovative approaches to provide families with a broad and integrated set of supports as they make the transition to new housing and to economic self-sufficiency. Urban Strategies also collaborates with other community organizations to increase the human capital-building assets in the revitalized community.

Job Summary:

Urban Strategies is seeking a Community Health Specialist to support the outreach and engagement of individuals and families in service of their health and well-being. The Community Health Specialist will support residents connected with the Comanche CNI, River West CNI and Country Club Gardens communities in Tulsa, OK. The Community Health Specialist will be responsible for sharing information with residents about health resources and services that will be of benefit. The position will be responsible for the program Community is Medicine, as well as support with community-wide events and activities, and public and resident meetings. The Community Health Specialist will gather pre-post surveys for measuring program outcomes and gaining input from the participants regarding improvements and possible changes to the program and services provided. The Community Health Specialist will assist the Project Manager and case management team with other tasks as requested. The Community Health Specialist will report to the Project Manager.

  • Major responsibilities include but are not limited to:

  • Manages and coordinates outreach events, health screenings, and other requests in partnership with health organizations.

  • Markets and promotes services and programs on a scheduled basis.

  • Strengthens existing relationships and cultivates new ones through working with existing health partners and new organizations in Tulsa, with a focus on North Tulsa

  • Coordinates professional presentations to community groups on educational topics.

  • Coordination and development of Community is Medicine program.

  • Maintain the established health plan regarding the development of relationships and contacts, resulting in increased referrals and overall community involvement.

  • Maintain documentation of outreach results.

  • Enter and track resident data entered into Urbans customized performance management system LEARN to document resident progress.

  • Maintain privacy and confidentiality of all household information given.

  • Actively participate in Urban Strategies program meetings and other meetings related to the project.

  • Be available to occasionally work nights and/or weekends, and travel.

  • Performs a range of associated administrative functions, as appropriate, such as monitoring budgets, preparing administrative and programmatic reports and correspondence, and submitting billing documentation, as required.

  • Performs miscellaneous job-related duties as assigned.

Qualifications:

  • Bachelors degree in public health, human services, or related field, relevant job experience, or a combination.

  • Experience in Microsoft Outlook and Microsoft Office software.

  • Ability and commitment to handle privileged information in a professional and confidential manner.

  • Availability to attend evening and weekend functions as required.

  • Ability to function as a team player in both internal and external relationships.

  • Strong verbal and written communication skills.

  • Ability to lift and carry up to 25 lbs.

  • Ability to walk up and down stairs when door-knocking in the community.

  • Possession of a valid drivers license and availability of automobile for work use

  • Must be proficient in Microsoft Outlook, Word, and Excel

Skills and/or competencies required:

  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.

  • Strong organizational, planning and time management skills.

  • A strong sense of and respect for confidentiality involving both participants and fellow employees.

  • Must excel in fast-paced environment

  • Ability to work in a variety of settings with culturally diverse persons and communities with the ability to be culturally sensitive and appropriate.

  • The ability to work independently and with initiative, combined with skills for thriving in a team environment to achieve shared goals.
