Posting Summary
Rutgers, The State University of New Jersey is seeking a Community Program Specialist for the Rutgers Cancer Institute of New Jersey. The Community Program Specialist will implement and coordinate SDOH interventions across various organizational entities and in partnership with external stakeholders. This position supports program administrative functions, participant engagement, patient/community outreach and data collection and reporting. The Community Program Specialist will also serve as a member of the ScreenNJ Prevention Care Team, working within the community setting, providing outreach to patients with medical and social needs. The Community Program Specialist acts as a resource to assist patients, in navigating the health system; and serves to facilitate linkage to community resources.

- Serve as a resource for patients and community to connect with needed SDOH-related services (community health, social services and transportation).
- Design and maintain clear operational guides and processes to ensure program efficiency.
- Coordinate program activities, support daily operations, set program priorities and maintain regular communication and collaboration across programs.
- Integrate social determinants of health strategies into contracts, proposals, and workflows to advance platform effectiveness and reach.
- Organize transportation through standard Rutgers procurement process working directly with department Director and support staff team.
- Maintain relationships with transportation vendors, patients, community members and agencies.

COVID-19 Immunization Requirement
Under Policy 60.1.35, Rutgers University requires all prospective employees to provide proof that they are fully vaccinated and have received a booster (where eligible) against COVID-19 prior to commencement of employment, unless the University has granted the individual a medical or religious exemption. Prospective employees who are not eligible for a booster at the time of an offer of employment must provide proof they have received a booster within two weeks (14 calendar days) of eligibility. Based on current guidance, individuals are considered fully vaccinated against COVID-19 two weeks after receiving the final dose of any COVID-19 vaccine authorized or approved for use in the United States by the United States Food and Drug Administration (FDA) and recommended by the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention (CDC). Eligibility for a booster against COVID-19 varies and is explained on the University’s web site located at https://coronavirus.rutgers.edu/covid-19-vaccine. Failure to provide proof of primary vaccination and booster will result in rescission of a prospective employee’s offer of employment and/or disciplinary action up to and including termination.

Minimum Education and Experience
- Bachelor’s degree in social work, public health or related field.
- Minimum 3 years’ experience working in nonprofit, community services/outreach, public health or healthcare industry.
- Experience engaging with health insurance and medical health care providers.
- Equivalent experience, education and/or training may be substituted for the education requirements.
Certifications/Licenses
Required Knowledge, Skills, and Abilities
- Strong understanding and respect of all cultures and demographic diversity. Effective oral and communication skills.
- Must be computer literate with proficiency and working knowledge of database and reporting tools such as Microsoft Word, Excel, Access, and PowerPoint.
- Strong interpersonal skills and ability to build collaborative relationships.
