GENERAL JOB

DESCRIPTION

Provide individual counseling sessions and evaluation of members as needed

Conduct Caregivers Support Group and public education sessions.

DUTIES AND RESPONSIBILITIES

  • Provide individual counseling as needed to registered members. Conduct initial psychosocial assessment as well as ongoing general evaluations to develop an Individual Care Plan. Create and maintain confidential client files, including Individual Care Plan and progress notes
  • Maintain positive relationship with member’s social support in order to obtain feedback
  • Through observation and discussion, identify concerns and needs, and refer for additional Social Services as necessary
  • Research and implement new methodologies and approaches in gerontology and mental health counseling to create and facilitate therapeutic counseling for groups and individuals
  • Develop collaborative working relationships with community service providers; outreach
  • Facilitate Caregiver Support Group (weekly)
  • Provide Public Education series covering wide range of topics (including but not limited to Emotional Development, Communication with Family Members, Dealing with Stress, Self-Esteem, Personal Development)
  • Track units, and provide monthly report to Administration
  • Participate in UR (Utilization Review) meetings to problem solve/address conflicts involving members
  • Provide staff support if necessary to improve communication with members
  • Participate in Staff meetings scheduled by Administration
  • Participate in ongoing training as required to fulfill duties and responsibilities
  • All other duties as assigned including registering of new members and
  • updating memberships.

SKILLS/EXPERIENCE/ EDUCATION

  • Bachelor’s level degree or above in Clinical Mental Health Counseling, Social Work, Marriage and Family Counseling, Psychology, or other relevant field with experience or training in geriatric service provision
  • Knowledge of programs and services offered by the Center
  • Knowledge of community programs and services
  • Ability to make accurate determinations through observation
  • Positive and friendly demeanor
  • Ability to remain calm and composed in emotional setting

COMMUNITY SERVICES

  • Ability to work as member of a team
  • Ability to maintain confidentiality
  • Computer skills and ability to learn variety of programs
  • Organizational skills
  • Bilingual preferred

REVIEW

  • Ongoing informal review, with annual written formal review
