LPN or RN for Enhanced Services

Physical location is close to Milesburg, PA
Do you want to have a direct impact in someone's life and help them to live life to the fullest under difficult circumstances? This position will directly help an individual with intellectual disabilities and their family in the environment they are comfortable with from their home.
We are looking for an experienced individual to fill the role of Support Service Professional.
The right candidate will be responsible for providing compassionate care and assistance to a woman with intellectual, developmental, and physical disabilities to participate in an everyday life in the community. All candidates must possess a strong belief in the participant self-direction model. If you have prior experience working with developmentally disabled adults in a direct care role, we encourage you to apply for this position.

$35hr - Mostly Second Shift 1:30pm-8:30pm but need to be flexible to cover first shift also. Every other weekend availability needed first or second shift.

Need flexible schedule to fill in for other shifts as needed.

Duties include:

• Assist with the implementation of the outcomes in his Individual Service Plan (ISP)
• Promote home and community based safety and welfare
• Observation and documentation of mental and physical changes and provide care when she exhibits these changes.
• Assistance with exercises and any other therapies required.
• Promote and assist to identify opportunities for socialization through entertainment that leads to the development of friendships with people her age.
• Assistance with improving and maintaining her Daily Living Skills and homemaker responsibilities
• Complete daily documentation and reports
• Perform any reasonable function as requested by the individual, her family, or support team.
• Assistance with personal care needs
• Light housekeeping

Qualifications:Must be at least 18-years old 2+ years experience preferred Clear criminal background checks, which may include the PA Criminal Background Check and FBI Finger Print Check Possession of a valid driver’s license with a safe driving record Ability to work independentlyAbility to work effectively as a member of a team

• Ability to communicate effectively both orally and in writing
• Ability to lift 50 lbs or more
• Basic cooking skills with the ability to follow recipes
• Maintain a safe home environment
• Kneel, bend, squat, and reach frequently.
• Computer skills required
• Strong work ethic and dependability

Pay: $34.00 - $35.00 per hour

Ability to commute/relocate:

• Lock Haven, PA 17745: Reliably commute or planning to relocate before starting work (Required)
