Position Details: PRN (as needed) Home Health Resource Center Description:

To provide aide services to patients in their homes.

Principle Accountabilities: % Of Time Spent

1) Follows the written instructions for patient care as prepared by the Registered Nurse. 10%

2) Performs simple procedures as an extension of therapy services. 10%

3) Gives personal care to patients; including, but not limited to, bathing, shampoo, shaving, mouth care and skin care. 15%

4) Assists patient with ambulation, transfers, and exercise. 10%

5) Performs household services essential to health care at home. 5%

6) Cares for patient's surroundings as necessary to maintain a safe environment. 10%

7) Gives medication reminders on medications that are ordinarily self-administered. 10%

8) Reports to the Registered Nurses/Nursing Supervisor changes in the patient's condition and needs. 10%

9) Completes appropriate reports according to Homecare Agency policies. 5%

10) Attends all mandatory in-service programs: must have 12 hours of inservice per year 5%

11) Attends case conferences and staff meetings. 3%

12) Complies with Homecare Agency Compliance Plan. 2%

13) Complies with Homecare Agency's Infection Control Policies. 3%

14) Complies with Homecare Agency's HIPAA Policies. 2%

15) The Home Health Aide is primarily responsible to the Registered Nurse in charge of their patient assignment.

Education:
High School diploma or equivalent, or four years of direct experience in home care.

Experience:
A minimum of one year of Homecare experience is preferred. 2 years of hospital, nursing home or public health nursing is preferred. Satisfactory completion of the State Home Health Aide Competency Examination will be completed during departmental orientation. Must be able to read, write, and communicate well. Must be able to document patient care on appropriate agency form. The assessment, care and treatment provided by the Home Health Aide will be consistent with the specific age related needs of the patient. The Home Health Aide is competent to care for patient age groups including the geriatric patient.
