Job Summary

The Household Coordinator is a blended role whose primary purpose is to enhance the resident s quality of life by effectively managing the household. The Health Coordinator works closely with the Clinical Care Leaders, and Lead Trained Medication Aide (TMA) and has 24-hour responsibility for the non-clinical functions of the household. The Health Coordinator is also instrumental in carrying out consistency in documentation and processes between all households under the direction of the Administrator, Director of Nursing (DON), and other department managers. Primary responsibilities include the oversight of environmental services items throughout the Long Term Care (LTC) facility including daily, weekly and monthly room cleans, terminal room cleans upon resident discharge as well as special projects as assigned. The Health Coordinator plays a key role in orientation of new staff, the resident admission process, and communication of changes in the residents' daily needs to staff caring for the residents. The position meets universal expectations outlined in the Resident Assistant job description for all household staff. Health Coordinator staff are expected to know and will be held accountable for following infection prevention and control policies.

The Household Coordinator Participates in the admission process, transfers, room changes and discharge arrangements. Develops positive resident relationships and supports families involvement. Assists in investigations and reporting related to resident care. Assists, DON, Social Worker and Recreation Coordinator, and Manager, Nursing Long Term Care with administrative duties and tracking and documentation of resident activity and interaction. Ensures that resident mail is

distributed as needed per household. Assists in the planning implementation of facility events. Participates in care conferences for household residents as needed. Gives care plan input to Clinical Care Leaders, and communicates care conference results with staff as needed. Is knowledgeable about care plan problems and the measurable goals and pertinent approaches reflective of the current status of the resident. Ensures implementation of the care plan to front line staff for each individual resident in the household. Evaluates resident progress towards goal accomplishment and suggests updates to the care plan accordingly. Implements non-nursing interventions based on identified goals. Adheres to the facility guidelines for documentation that is written legibly and reflects approved abbreviations. Coordinates consistency in documentation between all households as directed. The Household Coordinator tasks also include but are not limited to meeting the patient's activities of daily living needs, meal preparation needs, emotional and spiritual support needs, medication compliance needs, and laundry.

In addition to administrative tasks and coordination the Household Coordinator will also provide support in the function of a Certified Nursing Assistant (CNA).

Qualifications

The Household Coordinator must have a minimum of five years' experience in long-term care.

Certified Nursing Assistant on the Minnesota Nursing Assistant Registry and Trained Medication Aide certification required.
