The Behavioral Health Lead Clinician reports directly to the Navigation Center Supervisor. Clinician provides individual triage, crisis, assessment, and referral coordination services for those experiencing moderate to severe mental health symptoms and conditions. In absence of supervisor, serves as primary contact and support for staff, clients, and community partners.

We are a Federally Qualified Health Center (FQHC) and have been providing care to Yolo County residents since 1972. Like other community health centers, we provide care to anyone that walks through our doors and are located in all major cities within Yolo County (Davis, Woodland and West Sacramento) and provide services throughout Yolo County. The community health center movement was born out of the civil rights movement in the 1960s with the mission to provide healthcare in communities where there are few or no options for care. Our mission is rooted in the philosophy that health care is a right, not a privilege. We serve and provide care for those disproportionally impacted by the social determinants of health.

SALARY:
- Exempt position
- Starting pay within this range will vary depending on the job-related knowledge, skills, and experience. The pay range for this position is:
- ASW/AMFT/APCC: $64,480-$73,500 annually
- LCSW/LMFT/LPCC: $76,500-$85,000 annually
- May be eligible for annual 2% + 0-2% merit-based pay adjustment (0-4%).

Schedule: 40 hours per week M-F, 1 Saturday rotation per month. Primary site is in Davis, CA, some travel to Woodland and West Sacramento may be required.

Essential Functions*

Clinician is a member of the Navigation Center team and is responsible for: responding to crisis, triaging client needs and conducting comprehensive biopsychosocial assessments for clients experiencing moderate to severe mental health symptoms and conditions, care coordination and referral, understanding billing structure, and how to correctly utilize and interpret ICD10 and DSM5 codes and criteria.
- In the absence of Integrated Behavioral Health Supervisor, Lead Clinician is responsible for day-to-day operations of Navigation Center and own job duties. May include staff support, clinical consultation, and decision making.
- Communicate proactively with supervisor, support with problem solving and program development efforts, and serve as role model for professional and mission driven conduct.
- Represent CommuniCare and program at community events, meetings, and workgroups.
- Collaborate directly with program case managers, peer partners, supervisor, community stakeholders, and the Yolo County Department of Health and Human Services to coordinate long term follow-up and care.
- Complete clinical documentation within 48 hours of services in accordance with working hours and agency, county, and contract requirements.
- Provide culturally responsive group counseling, assessment, and activity services for diverse populations with a wide range of needs/acuity.
- Monitors patient charts to ensure the timely placement of all correspondence, completes all screening and data collection tools, tracks caseload, and completes discharges according to protocol and best practices. Participate in quality assurance and improvement efforts.
- Manage complex caseload, direct service, and administrative duties in fast-paced setting.
- Respond to and manage crisis situations according to agency guidelines and legal/ethical mandates, including assessing and writing 5150 holds.
- Works with clients to establish short-term goals and safety planning. Clinician is responsible for detecting emotional, psychological, and physical risk factors and danger signals. Clinician is non-judgmental and offers feedback and support to clients.
- Provides information to clients including treatment alternatives, referrals, and other community resources.
- Responsible for remaining within the legal/ethical standards, including confidentiality and HIPPA.

- Responsible for understanding, explaining and enforcing all rules and regulations associated with clinic policy.
- Compile and report on outcome data as required.
- Attend clinical supervision/consultation, weekly staff meetings, meetings with community partners, training as arranged/required.
- Maintains current license/registration in good standing, submits all CEU and training certifications to HR.
- Responsible for maintaining the standards as listed in the_ CommuniCare Health Centers Policies and Procedures_, and any other standards which may apply to this position.

- May require direct transportation of clients as part of navigating services and appointments.
Education & Experience: LCSW/LMFT/LPCC preferred. Will consider ASW/AMFT/APCC registered with BBS with strong experience. Applicants must have a strong background in mental health assessment and crisis management.
Special Skills/Equipment: Bilingual preferred, Spanish/English strongly preferred. Computer skills to include Microsoft Office (Word, Excel, Outlook). Must provide current driver license and auto insurance, ability to travel in Yolo County (West Sacramento, Davis, Woodland).

Able to work independently with little or no direct supervision. Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external stakeholders. Must have a commitment to excellence and dedication to serving at-risk and vulnerable populations including those experiencing: homelessness, mental illness, cognitive delays, legal involvement, and extreme poverty. When safe to do so, transporting clients may be required.
