Be part of the opening and foundation of a unique facility and organization. This individual will have the rare opportunity to mold the department, to open a new facility and grow within the facility and culture. Health Center - Salem will transition the long term care institutional culture into a household community. We are looking for a team who can see our vision and assist in implementing it. The facility will have a 40 bed Rehab (skilled) ident directed care.
Responsibilities:
- Build and manage a successful MDS department
- Train and develop MDS Coordinators and Clinical Coordinators
- Manage the care management process from admission to discharge. This will include; MDS assessments, CAA’s, comprehensive care plans, direct communication with residents and team members.
- Accurate ICD-10 coding
- Auditing of assessments, care plans, schedules and documentation.
- To communicate professionally, clearly and effectively to the IDT, residents, families and medical providers.
Qualifications:
- RN licensed in VA
- 3-5 years of MDS 3.0 experience in a skilled nursing home
- RAC-CT preferred or ability to obtain within 6 months of hire
- Strong knowledge of PDPM, Medicare, Virginia Medicaid plans and case mix
- Ability to assess, plan implement and evaluate all aspects of care to include; physical, mental and emotional status and implement all required interventions.
- Strong working knowledge of ICD-10
- Strong understanding of quality measures, CMS star rating
- Possess excellent oral and written communication skills
- Possess long-term care leadership and management experience
