We are seeking an experienced and diligent Medicaid Coordinator to join our skilled nursing facility team in Amityville!

Responsibilities Include but are not limited to:

• Process of Medicaid applications, conversions and recertifications.
• Meeting with resident and/or family members and review all financial documents necessary to process Medicaid applications.
• Redirecting of income (NAMI) and monitoring that all necessary forms are completed and submitted to financial dept.
• Prepare all documents for necessary meetings with supervisor and other facility staff to review resident financial profiles.
• Strong familiarity with all aspects of current NYS Medicaid regulations, specifically Nassau and Suffolk County DSS.
• Meet and coordinate with residents and their families to discuss and advise financial obligations and coordination of documents.

Qualifications Include:

• Comfort and familiarity working with elderly/sick population as well as their families.
• Minimum 2 years’ experience compiling and submitting Medicaid applications
• Long-Term Care Facility strongly preferred.
• Proficient in Microsoft Excel, Outlook and Word.
• Proactive, detail orientated and extremely organized.
• Must possess a strong ability to multi-task and work effectively with staff, residents and families.
