Job description
Seeking a full-time Mental Health Care Manager. Expected working hours are Monday through Friday from 9am-5pm.
The Mental Health Care Manager is responsible for coordinating and supporting mental health care within the clinic and for coordinating referrals to clinically indicated services outside of the clinic. Mental Health Care Manager will serve as a liaison between the patient’s health care provider and mental health team.
Job requirements:
- Bachelor’s degree in Social Work, Psychology, or related field.
- Excellent interpersonal and listening abilities.
- Excellent verbal and written communication skills.
- Problem solving, decision making, and critical thinking abilities.
- Ability to stay calm during high-pressure situations.
- Ability to work within an interdisciplinary team.
- Ability to work with young people and their parents/guardians.
- Empathetic, patient, and understanding.
- Must be able to maintain client’s confidentiality and adhere to ethical practices.
- Must be able to relate with people from diverse lifestyles and cultures.
Mental Health Case Manager responsibilities include:
- Conduct interviews with patients as well as their families in order to review and assess mental health needs.
- Offer support and information to patients and their families.
- Devise a mental health plan of action based on patient’s individual needs.
- Coordinate patient’s individual needs with providers and mental health team.
- Develop crisis management plans.
- Help patients access community resources.
- Maintain accurate records.
Compensation:
- $20-$22/hour based on experience
