Nurse Practitioner and Physician Assistant

Full-time salaried position

AllCare Hospitalist Medical Group (AHMG) is a cutting edge health care delivery system affiliated with AllCare IPA. Our successful model of healthcare focuses on disease prevention and optimal patient interaction in order to keep people healthy and out of the hospital. We are looking for an enthusiastic and detail-oriented Nurse Practitioner (NP) or Physician Assistant (PA) to join our team to make positive impacts on the wellness of our Medicare population. Our clinics are located in San Joaquin, Stanislaus and Merced counties.

Responsibilities:

• Conduct Annual Wellness Visits with head-to-toe physical exam

• Address all health conditions with recommendation on care plan

• Reconcile medication list, review and update pertinent history

• Order lab and preventative screening tests

• Review and communicate test results with patients

• Provide wellness and disease management education to patients

• Complete documentation timely and accurately

• Provide patients with excellent visit experience

Qualifications:

• Active California PA/NP license and DEA

• Spanish Language skills preferred
