Provides customer service and administrative support to patients at check-in before they are seen by the providers to verify registration information on file and tomake time-of-service payments when appropriate.

Essential Duties & Responsibilities:

1. All patients will be asked to review and correct, if necessary, the information provided on the registration form. The patient will sign the form, confirming that the information provided is complete and accurate, and return it to the front-desk staff.

2. Patients with insufficient insurance information at the time of service will be registered and billed as “patient responsible” for charges incurred.

3. Practice staff will enter any changes to demographic and insurance information into the practice management system immediately. The most current signed registration form will be filed or scanned in the patient’s medical record, in the registration section.

4. New patients will be asked to provide a valid insurance card and/or identification at the time of the scheduled appointment. A photocopy or scanned image of the front and back of the card will be made and maintained.

5. If there are any changes to the registration information, a new encounter form will be printed and/or the registration information manually updated on the previously printed encounter form.

6. An encounter form will be attached to the patient chart and forwarded to the appropriate staff for treatment.

7. Co-payment needs to documented on the patient encounter form.

8. When the patient has completed the registration process, the patient will be arrived in the appointment scheduling system according to the practice management system protocol; or the clinical staff will otherwise be informed of the patient’s arrival. This announcement may occur prior to keying in the registration information arrival. This announcement may occur prior to keying in the registration information for that patient, but after the patient has completed registration responsibilities.

9. All other duties as assigned.


High school diploma or GED.

1. Ability to establish and maintain courteous and professional working relationship with other staff and patients.
