1.Demonstrates the ability to improve patient care by the consistent application of professional knowledge and skills to solve drug therapy related problems.
a.Dispense medications and follow the institution's and department's policies and procedures, utilizing pharmacy technical support and assisting with inventory control.
b.Establish a collaborative working relationship with healthcare providers; design, recommend, monitor and evaluate patient-specific therapeutic regimens and document patient-care activities appropriately.
c.Conduct medication use evaluations prepare and present evaluations for formulary addition requests via Pharmacy and Therapeutics Committee.
d.Provide two continual educational programs to pharmacy staff, provide instruction to pharmacy technicians, students and other pharmacists, co-precept pharmacy students on rotation.
e.Provide concise and timely responses to drug information requests; review current literature to keep up to date on new drugs, indications, and medication safety alerts.
f.Prepare drug information responses, chart notes and submissions to our pharmacy newsletter.
g.Communicates clearly when speaking or writing. Organizes all written or oral communication in a logical manner. Addresses all communication at the level appropriate for the audience as evidenced by peer evaluations.
h.Demonstrates competency in specific clinical services including pharmaceutics, renal dosing, anti coagulation therapy and others as identified.

2.Clinical Staffing Requirements
.The resident will document hours spent in the residency program in an effort to assure that ASHP requirements are met. These standards have been established for the benefit of patient safety, provision of fair labor practices and the minimization of risks of sleep deprivation.
a.Resident will adhere to on-site hour limitations (no more than 80 hours per week) and will be off-site at least one day per seven day period.
b.Resident will document compliance with these monthly standards monthly through e-mail communication to their Program Director and Pharmacy Director.
c.Provides education and drug information to practitioners and patients utilizing effective communication skills.
d.Provides in-service education to physicians, nurses, and other practitioners. Providers instruction to pharmacy technicians, pharmacy students, and pharmacists as evidenced by acknowledgements and training and attendance logs.

3.Participates in quality improvement efforts.
.Participate in the following: development or modification of policies for medication use, assessing clinical interventions and other quality improvement programs.
a.Assist Medication Safety Committee in reviewing quarterly medication errors, review and update Joint Commission policies and procedure, and evaluated drug administration and other safety issues including high-risk drugs.

4.Meets all goals and objectives as described in the residency manual.
.Conduct one primary research project suitable for presentation at the Eastern States Residency Conference.
a.Attend ASHP Midyear Clinical meeting.
b.Participate in National Pharmacy Week.

Qualifications


Experience Minimum Required

  • Clinical experience commensurate with PharmD program requirements, fulfillment of internship hours.

Education Minimum Required
  • Doctorate of Pharmacy degree

Training Minimum Required
  • PharmD degree, letter of intent, college transcript, curriculum vitae, 3 letter of recommendation, personal on-site interview.

Special Skills Minimum Required
  • Basic knowledge of drug therapeutic and performing pharmaceutical calculations; good oral and written communication skills, computer skills (creating Word documents, PowerPoint slides, Excel).

Licensure and/or Certifications Required
  • New Hampshire Intern License
  • New Hampshire Pharmacist License within 90 days of hire.
