This Pre-Visit Care Connector position is responsible for coordination of care prior to a HouseCalls Visit to ensure the documentation of accurate and up to date information. Care Connectors also assist with communication to Providers (both internal and external), as well as members and their families during and after their HouseCalls visit. The HouseCalls Care Connectors will work in a call center environment virtually from their own home to provide immediate care coordination needs via telephonic incoming calls.


This position is full - time, Monday - Friday. Employees are required to work our normal business hours of 8:00 AM - 8:00 PM CST (Central Standard Time zone).


We offer approximately 4 days of training. The hours during training will be 9:30 AM - 5:30 PM from Monday - Friday. Training will be conducted virtually from your home.


  • All Telecommuters will be required to adhere to UnitedHealth Group’s Telecommuter Policy.


Primary Responsibilities:


  • Function as an advocate both pre and post HouseCalls visit

  • Educates member on gaps in care and assists with scheduling provider appointments

  • Ensures documentation in EMR utilized by HouseCalls Practitioners is up to date and accurate prior to the visit to assist with a smooth and engaging yearly visit

  • Gathers and charts demographic data from members prior to the visit

  • Completely and accurately document in patient’s electronic (UnitedHealth Care) medical record

  • Provide patients and family members with education regarding the need for follow up as appropriate

  • Actively participate in organizational quality initiatives

  • Demonstrates a clam, organized and courteous demeanor telephonically to members and Practitioners

  • Demonstrate a commitment to the mission, core values and goals of UnitedHealthcare and its healthcare delivery including the ability to integrate values of compassion, integrity, performance

Required Qualifications:


  • High School Diploma / GED (or higher)

  • 2+ years of experience in a healthcare - related field, which includes working with medical terminology and / OR taking patients histories

  • 1+ years of call center experience (Inbound and Outbound)

  • Ability to work any 8-hour shift between the hours of 8:00 AM - 8:00 PM CST (Central Standard Time zone)


Preferred Qualifications:


  • Bachelor's Degree (or higher) in social work, licensed practical nurse (LPN), CNA / HHA, OR High School Diploma / GED with 3+ years of experience with working within the community health setting in a health care role

  • Experience with working in managed care

  • Bilingual fluency in English and Spanish

  • Certified Nursing Assistant / Home Health Aide, OR Medical Assistant


Telecommuting Requirements:


  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy

  • Ability to keep all company sensitive documents secure (if applicable)

  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

The salary range for California / Colorado / Connecticut / Nevada / Washington / New York City residents is $18.80 - $36.78.
