The SASS program is a family-based alternative sentencing partnership between Washtenaw County, the Office of the Public Defender, the Prosecutor’s Office, the Washtenaw County Trial Court to achieve the goal of diverting young people who have entered the criminal justice system from ending up with a criminal conviction and the subsequent negative consequences. This pre-plea diversion model supports a harm reduction approach, offering an individualized program plant that may consist of but is not limited to behavioral health services, mental health services, physical health services, job training and education assistance, and mentorship.

Job Summary: Under general direction with a high level of autonomy, uses extensive knowledge and skills obtained through education and experience, to manage the Specialized Alternative Sentencing Support (SASS) program at The Corner Health Center. This is a working position requiring the Program Manager to also maintain an active patient load. Directs and manages the overall responsibilities of complying with all grant-requirements. Leads program and processes with significant scope and impact involving cross functional teams. Provides comprehensive solutions to complex problems or needs. Results are evaluated at agreed upon milestones for achieving goals and objectives. Extensive contact with internal clients and external business partners is needed to meet goals and objective.
Essential Duties and Responsibilities:
· Establish and maintain positive relationships between Corner Clinic Providers and the Washtenaw Trial Court.
· Work with the Washtenaw Trial Courts to facilitate the intake, assess and evaluate program participants and report back to the Court regarding participants progress in achieving expected benchmarks.
· Develop customized participant programs with identifiable benchmarks and regularly track and monitor progress.
· Facilitate weekly meetings and follow-up with action steps, including coordinating continuing education trainings and updating the team on internal policies and general updates.
· Demonstrates and encourages social workers to use a multi-disciplinary team approach to participant care by making cross-referrals to medical services.
· Work as part of an interdisciplinary care team member for complex cases for coordination of care.
· Identify essential participant resources and referrals for those that experience a crisis or in need of higher level of care.
· Provide support with interventions as needed by assessing urgent patient care needs, processing patient intakes, and conducting the Warm Hand-Off Process to provide immediate care.
· Explore and implement therapy groups (when applicable)
· Assist with maintaining/moving the SASS teamwork flow by assigning new participants to therapist, maintaining participant for each therapist, including records on level of care (length of service, how often service is needed, etc.), discharges (when to), and follow up. This includes managing the therapy wait list.
· Monitor team’s daily encounters and scheduling of regular appointments in order to meet productivity goals
· Track no-show appointments to support and improve continuity of care and treatment.
· Keep uniform records by reviewing and auditing participants files.
· Monitor team’s productivity, daily task completion, review process, disciplinary actions, performance enhancement plans, and follow up with clinicians when they are not meeting expectations
· Assist with handling patient complaints related to services provided.
· Identify, gather and provide data and information on a monthly and/or quarterly basis to the Executive Team, external grant manager, and grant funding agencies for accreditation reporting.
· Actively maintain a patient load of approximately 8 patients per week.
· Work with Clinic Director to comply with all clinic processes and procedures, including training and use of the EMR system.
· Interprets, applies and identifies program improvement needs
· Plans, assigns, monitors and manages the SASS program social work team including setting goals, expectations, outcome reporting and any other team needs.
· Reviews performance, evaluates results achieved, and coaches and mentors direct reports.
· Reviews progress of program with senior management.
· Balances conflicting resources and priority demands.
· Additional duties as assigned
Knowledge, Skills, and Abilities:
Knowledge of:
· Trauma-informed practice, with experience in cognitive behavioral therapy and at least one other evidence-based practice.
· Criminal justice diversion programs and best practices.
· Minor consent laws.
· Billing practices and procedures.
· Computer-based documentation of care.
Skill in:
· Preparing for grant-related audits
· Working in a community health/medical setting
· Operating standard office equipment and using required software applications including Microsoft Office, Teams, and electronic medical health record systems.
Ability to:
· Partner with other functional areas to accomplish objectives.
· Facilitate program and other integrated care meetings, ensuring that all viewpoints, ideas and problems are addressed.

Educational/Previous Experience Requirements:
· Minimum Degree Required: Master’s degree
· Required Disciplines: Social Work or related clinical field ~and~
· At least 3 years clinical experience working with adolescents and young adults, preferably diverse and underserved populations
Licenses/Certifications:
· State license (Michigan) LMSW required
Working Conditions:
Hours: Normal business hours. Some additional hours may be required

Working Environment: Climate controlled office environment during normal business hours.
Total Compensation and Benefits:
· Salary of $55,000 - $65,000, commensurate with experience
