Description:

Serves as the Project SUCCESS Counselor in the Lincoln Middle and High Schools. In collaboration with school counseling staff, and under the guidance of the Managers from Community Health Improvement and Behavioral Health Services, the Counselor will implement the evidenced based Project SUCCESS program, which targets risk and protective factors associated with youth substance abuse.

Essential Functions and Responsibilities:

  • Provide pre-assessment and referral information to students and parents who are affected by alcohol/drug use.
  • Conduct early intervention/insight programs for students experimenting with alcohol/drug use or suspended for alcohol/drug use
  • Consult with the staff or the school on how to identify, assess, motivate and refer students who are high risk
  • Coordinate referrals to mental health, substance use disorder treatment programs and other community services
  • Conduct specialized individual and group counseling for high–risk students (recovery, COA, intervention, new comers and others, coping skills, etc)
  • Work collaboratively with the school and community in bridging the gap between community and school services for students
  • Maintain program records and files, compile data and prepare reports as needed to comply with funding requirements
  • Enter data as required into appropriate systems
  • Manages multiple tasks and project initiatives.
  • Plans and implements project activities within budgetary limits.
  • Treats all families, co-workers and visitors in a caring and respectful manner, being mindful of individual differences, and cultural and ethnic diversity.
  • Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participates in their resolution.
  • Maintains the confidentiality of information acquired pertaining to students, families, patient, physicians, employees, and visitors to Chelsea Hospital, Trinity Health Ann Arbor, and the Lincoln School District. Discusses student, patient and hospital information only among appropriate personnel in appropriately private places.
  • Maintains knowledge to perform the duties of the job. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
  • Behaves in accordance with the Mission, Vision and Values of Chelsea Hospitalh.
  • Identifies and assists in resolution of problems related to safety, sanitation and maintenance of the environment.
  • Practices effective communication in all interactions (patient, visitor, co-worker, etc.)
  • Promotes a positive work environment by demonstrating teamwork. Demonstrates self-directed learning and participates in ongoing professional development. Demonstrates responsibility and accountability for own professional behavior.

Qualifications:

  • Master’s degree in social work, counseling, psychology or related field.
  • LMSW, LPC, LLP – State of Michigan is preferred.
  • MCBAP Prevention Certification is preferred; a developmental plan must be on file with MCBAP within 30 days of date of hire.
  • Strong community outreach, organizing, and engagement skills.
  • Proven track record in community organizing and mobilization including education and volunteer experiences.
  • Excellent interpersonal and communication skills
  • Knowledge of prevention and substance use disorders.
  • Minimum of 2 years' experience in substance abuse/mental health counseling.
  • Experience working with the adolescent population.
  • Experience working in a school setting.
  • Ability to work independently.
  • Familiarity with Microsoft Office products including Word, Excel, Outlook, and PowerPoint required.
