International Quality Homecare of WI is seeking Per Diem RN Case Manager for our Hayward, WI branch office. We offer remote work options (this means that cell phone & laptop provided to all Case Managers for you to use to complete your client visits in the field and no need to go to the branch office unless there is a meeting scheduled) as well as flexible scheduling based on client service needs.
Basic Qualifications:
The RN Case Manager position requires a minimum of an Associate degree in Nursing (RN) with at least 2 years experience in home health or Nursing home. The Nurse must be licensed to work in the state of practice. The Nurse is expected to have a valid driver’s license and a good driving record.
Other Qualifications:
The position requires a clear understanding of nursing in-home care settings. The ability to assess the needs of the clients and work with individuals within and outside the company to meet the needs of the client is required. The position requires a demonstration of strong analytical, problem-solving, communication, and leadership skills. Computer proficiency and the ability to learn different computer programs are highly required. Supervisory experience is preferred but not required.
Job Description:
The Nurse Case Manager provides nursing care to the clients. The Nurse Case Manager works with stakeholders within and outside the company to ensure that the needs of the clients are met.
Responsibilities:
Prepares OASIS and other medical documents. Provides nursing care to the clients.
Field Supervision of Field Staff communicates concerns regarding field staff to Operations Coordinator for performance management and any necessary disciplinary action.
Resolves Nursing and Administrative issues.
Works with Caseworkers, Counties, Hospitals, and other parties that are involved in the case.
Represents the interest of the company, client, and field nurses.
