Home Health RN Case Manager responsibilities include:

• Coordinating all aspects of the interdisciplinary care and services provided to hospice patients and their families.
• Provides education to families and caregivers
• Develops, implements and reviews healthcare plans
• Advocating for patients and their families
• Documentation of care
• Nursing assessments
• Symptom management
• Promoting comfort and quality of life.

Job Requirements:

• Current RN license to practice in the State of CO.
• Current CO driver’s license
• Reliable transportation
• Covid Vaccination Complete
• Maintain current CPR certification
• Excellent communication skills
• Minimum of two years nursing experience with at least one year in hospice or home health preferred, but not required.
• Self-directed and able to work with minimal supervision.
• Demonstrate excellent observation, problem solving, verbal and written communication skills, nursing skills per competency checklist.
• Shows ability to organize and prioritize workload independently.
