Job Title: Service Coordinator

Reports to: Program Manager-Housing

Position Summary:

The Service Coordinator provides oversight and case coordination to clients residing in our permanent supportive housing program in Placer County. The Service Coordinator will work in partnership with the Peer Counselor to assist clients in establishing goals, track client goal progress and strive to move clients in the direction of independence and self-sufficiency.

Essential Job Duties:

  • Complete program intakes and discharge paperwork for residents
  • Provide on-going support and case management expertise to residents in our permanent supportive housing program and ensure all resident case management plans are current and address identified needs. Continually monitor and evaluate each resident’s progression through their case management plan, and update as needed
  • Advocate for residents for public entitlements and housing opportunities
  • Coordinate services with County and community service providers and maintain regular, ongoing contact with County identified case manager
  • Assist in crisis management and resolution when issues arise with residents and support conflict resolution when residents are experiencing interpersonal challenges in the housing setting
  • Assist tenants with learning/improving daily living skills in order to help them develop life skills necessary for living independently
  • Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and AMIH guidelines including HMIS entries
  • Ensure annual paper work is completed on time and all documentation is submitted to the Data Analyst
  • Provide information and referrals and facilitate linkages for residents to services as needed
  • Actively participate in resident-related meetings and trainings
  • Attend program-related community, County or agency meetings as assigned
  • Adhere to strict boundaries and professional ethics in the care of others
  • Other duties as assigned

Qualifications Required:

  • Bachelor’s degree in social services related field REQUIRED
  • 2+ years experience with case management coordination for people with mental health, substance use and/or homeless histories, housing related experience strongly preferred.
  • Bilingual-Spanish/English STRONGLY PREFERRED with pay differential.
  • Experience with Coordinated Entry and HMIS preferred.
  • Experience collaborating and coordinating with County and community service providers
  • Experience working with persons with history of homelessness
  • Knowledge of Housing First Principles
  • Experience (2+ years) with housing subsidy programs such as: HUD Section Plus care, HUD Housing Choice, HUD Permanent Supportive Housing, and VASH Veterans Affairs Supportive Housing.
  • Knowledge of Tenant Rights and Responsibilities
  • Proficiency in Microsoft Office products and utilizing the internet.
  • A strong desire to help people who are homeless become independent successful members of their community.
  • Ability to exercise sound judgment, use discretion, and maintain confidentiality.
  • Ability to work independently and meet deadlines with average level of supervision, handle frequent interruptions, and maintain an atmosphere of teamwork.
  • Patience, with an ability to remain calm in difficult situations.
  • Have and maintain integrity at all times.
  • Personal vehicle and valid driver’s license, clean driving record, and personal automobile insurance.

Fingerprints, drug and alcohol testing, and criminal background checks are required pre-employment.
