The Team Registered Nurse is responsible for patient and family satisfaction through the provision of nursing care by making patient visits as assigned, conducting assessments, triaging patient needs, creating, following, and updating the plan of care, providing interventions, communicating, and documenting. The Team RN provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations, and policies.
$4000 sign on bonus- 1.0 FTE (40 hours/week)
- Full Benefits eligible
- 8:00 am - 5:00 pm
- Every 4th weekend rotation
- Pick up on call: Minimum of 2 shifts a year
- Patient and Family Assessments
- Assess patient and family physical, pain, psychosocial, emotional, spiritual, environmental and educational needs as outlined by the plan of care (POC).
- Collect data from the patient, family, significant other or other members of the interdisciplinary team and other health care professionals.
- dentify, interpret, and document assessment data collected in the nursing interview, physical assessment and other health data.
- Document and report significant changes in condition or unusual incidents to appropriate IDT member such as client abuse/neglect, suicidal intent, harmful coping mechanisms or violence, etc.
- Patient Visits
- Respond to patient and family needs or change in condition or need for acute patient intervention.
- Provide interventions, treatments, and pain management to patient to meet needs. Evaluate the effectiveness of pain management or other intervention and adjust to meet needs.
- Provide patient and family education on pain, symptom management, end of life, etc. Evaluate the effectiveness of education and adjust to meet needs.
- Plan of Care
- Read, understand, update, and follow the POC. Document interventions, outcomes, and changes in patient condition in electronic health record (EHR).
- Delegate visits and make referrals to appropriate disciplines and volunteers.
- Oversee work of Certified Nursing Assistants (CNA) and Licensed Practical Nurses (LPN).
- Miscellaneous
- Participate in meetings to plan and evaluate patient/family services.
- Precept new staff and students.

- Associate degree of Nursing or Bachelore's degree of Nursing
- Current, unrestricted Wisconsin RN license
- One year direct patient-care nursing experience, preferably in ICU, ER, Medical-Surgery, Oncology or Palliative Care or two years relevant health care experience.

- Valid Wisconsin driver license and car insurance and reliable access to an automobile in good operating condition.
