Job Description: Traveling Vaccinating Clinician

HealthX Clinicians work collaboratively as active members of the inter-professional HealthX team to provide professional patient and family centered care. They systematically and continuously collect and assess data related to a patient's health to develop, execute, and evaluate the proper plan of care. Clinicians utilize collected evidence to drive clinical decisions related to an individual or communities' health population. They establish goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The Clinician considers factors related to safety and fiscal accountability in planning and delivering care. Our Clinicians are in accordance with State and HealthX Professional Standards of Practice. The ideal candidate will participate in continuous quality improvement initiatives and assumes responsibility for their professional development.

MAJOR RESPONSIBILITIES

• Executes clinical interventions by collaborating with the inter-professional team incorporating appropriate standards of care and practice and patient outcomes
• Assesses and systemically collects any data related to patient health status and works in collaboration with the care team to develop the plan of care
• Evaluates patient’s response to plan of care and works in collaboration with the inter-professional team to make any needed adjustments
• Utilizes data to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers
• Assumes responsibility and accountability for effectively managing care of individual patients
• Documents nursing assessment and interventions to reflect comprehensive and integrated approach to patient care
• As part of the HealthX Professional Standards of Practice, identify learning needs of the patient and communities.
• Provides care that is safe, ethical, patient, family centered.
• Culturally congruent, and evidence based.
• There are skills and responsibilities that are common across the Clinical setting (primary or specialty care). Clinicians may perform any or all these skills and responsibilities. These may include, but are not limited to:
• Telephone, charting and in-person triage (for symptom management)
• Review and act on test results.
• Review and manage communications from internal and external “customers”
• Initiate and monitor vaccines.
• Serve as a mentor and resource to others.
• Assist with the coordination of care across the continuum (e.g., at-home, pop-up clinics, church events, schools).
• Assist with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness
• May float to other clinics on as needed for operations need

• Works independently within their scope of practice to achieve clinical outcomes
• Maintains a level of professional development through continuing education, quality improvement initiatives and sharing of knowledge
• Leadership through collaborative partnerships in accordance with the State and HealthX Professional Standards of Practice.
• Demonstrates competence within a continuum of professional development from advanced beginner to expert practitioner
• Utilizes evidence and science to guide practice

Hiring Locations: Savannah, Warner Robins, Dublin, Valdosta, Albany, Douglas, Waycross, Tifton, Vidalia, GA

Education:

Minimum: Graduate of an accredited clinical program (Paramedic, LPN, RN, or APRN)

Work Experience:

Minimum: One (1) year of clinical vaccination experience

Licenses & Certifications:

Valid registration or licensure of clinical accreditation in the state where employed

CPR/BLS Certification

Compensation: $30.00 - $40.00/hr.
