We are looking for a Home Health Intake Coordinator/Liaison to join our team to promote and educate professionals, facilities, organizations, and consumers regarding the services provided by Windsor Home Health.

You Will:

• Promote customer service by establishing working relationships with all referring entities in order to provide assistance in identifying patients who may be appropriate for home health services, thereby increasing the number of patients served by Windsor Home Health; maintains availability for referral sources to assist with patient referrals and expedite process.
• Act as a liaison between Windsor Home Health and the referral source (s) regarding patient care issues and concerns.
• Present Windsor Home Health programs and available services to civic and community professional groups, health care providers, discharge planners, case managers, trust officers, etc.
• Act as an educational resource to health care facilities, patients, caregivers, and physicians by providing both formal and informal educational programs on home health care coverage, programs, and services provided.
