You will support Inpatient / ICU / Trauma / A&E / Outpatient care by assessing psycho-social and financial needs, and providing counselling to patients including their families.

Your job scope includes close collaborations with the multi-disciplinary team in clinical setting and recommend appropriate interventions to support / facilitate a holistic care plan for patients to successfully integrate back home and into the community.

You will conduct financial assessment, field visit, build good rapport with government / community / social agencies or individuals to mobilize social resources to meet patient's needs. You will also be expected to initiate and implement support programs / improvement projects / learning initiatives for the Medical Social Service Department.

• Degree or Postgraduate Diploma in Social Work is required
• At least 3 years of social work experience preferably in clinical setting
• Strong advocate for holistic and patient-centered approach to care of patients with medical condition(s) and/or disability
• Must be a team player who enjoys working in a fast paced environment
• Being a registered member with the Singapore Association of Social Workers
