Position Summary: Recuperative Care Team: Will work with homeless individuals at 40 Prado needing 24-hour supervision due to recent discharge from a participating hospital or nursing facility. Will work with the team and all other shelter services staff to provide clients with needed shelter services aimed at stabilizing their physical health needs. Will assist clients in maintaining their physical health, including assistance with activities of daily living.
Works with emergency services personnel, such as law enforcement and paramedics. Supports CAPSLO’s mission by providing care and assistance to clients, encouraging and facilitating self-sufficiency. Shelter Care Worker: Identifies and provides direct services to homeless individuals and families at the 40 Prado Homeless Services Center. Services may include assessment, information and referral, distribution of needed basic supplies, as well as on-site client supervision.
Works with multiple service providers to enhance, augment, and coordinate service delivery to programs. Works with emergency services personnel, such as law enforcement and paramedics. Supports CAPSLO’s mission by providing care and assistance to clients, encouraging and facilitating self-sufficiency. Required Education and Experience: Requires a combination of four years education and/or experience providing direct human or health) services to the economically disadvantaged, physically/mentally disabled, substance abuse/dual diagnosed individuals, the elderly, and/or at risk youth.
Education, volunteer experience and/or other related experience may be accepted. Essential Functions: Recuperative Care 1. Together with the RCP Case Manager, RN and Shelter staff, assists in the completion of intake and program assessment; development of care plan with short and, if appropriate, long term goals and the steps to achieve those goals. Manages Homeless Services documentation including; client messages, daily log, client warnings, incident reports, CWS reports, filing, and other clerical duties as required.
2. Maintains a working understanding of the issues surrounding chronic homelessness and medical care issues including; Post Traumatic Stress Disorder, Traumatic Brain Injury, alcohol and drug dependency, and other mental health issues. 3. Adheres to laws regarding confidentiality and reporting requirements; maintains knowledge of current HIPAA certification standards.
4. Works closely with Case Manager and RN to ensure timely follow-through for all necessary medical, mental health, drug/alcohol appointments. 5. Maintains a professional relationship with clients and conducts all business related activity according to program policy.
Monitors client behavior regarding compliance 6. issues and follow-up; provides consistent limits and expectations regarding program policies and goal achievement. Maintains a safe environment by continually monitoring the immediate site and premises. Maintains an assault free environment for the protection and safety of clients, staff and visitors by utilizing preventive measures.
Makes emergency phone calls as necessary. Communicates with clients and shelter staff daily. Communicates with RN, at minimum, twice weekly. 7.
Assists clients in moving out of Recuperative Care program, once medically appropriate, and into daily shelter population. Communicates with shelter staff and homeless services case managers to ensure smooth transition. Shelter Services Worker: 1. Completes all required logs, timesheets, reports and client documents as required.
2. Maintains statistical information for each individual on a daily basis. 3. Coordinates and oversees ongoing client housekeeping activities to ensure clients are clean and safe at all times, especially prior to clients entering into program premises.
Addresses maintenance and housekeeping issues as they occur. Distributes personal and medical hygiene items to clients. 4. Provides, coordinates and oversees food service and coordinates meal provider staff regarding meals and food donations.
Maintains proper sanitation practices, clean-up, pick up and delivery of food. 5. Orients and interacts with various service providers and community members. Assists, as needed, in training and providing direction to other staff and volunteers.
Responsibilities and Duties: 1. Attends staff meetings and training sessions as required. 2. Must use reasonable precautions in the performance of one’s duties and adhere to all applicable safety rules and practices; and act in such a manner as to assure at all times maximum safety to one’s self, fellow employees, clients and children.
3. Performs other duties as needed. Knowledge, Skills and Abilities: Knowledge 1. Must be able to use Microsoft Office Programs and Outlook for data entry, documentation, reporting, logging and essential communication.
2. Must become proficient in ClientTrack and HMIS within one month of hire. Skills 1. Must be capable of working with an at-risk population for health and safety issues, including those with a previous history of involvement with the criminal justice system.
Abilities 1. Must demonstrate ability to interact and supervise medically fragile adults within a fast paced environment that includes a demanding and diverse population. Must be willing to assist clients with activities of daily living at Level 1 – 3 (verbal assistance/stand alongside client) for eating, transferring in/out of bed/chair/toilet. 2.
Must be able to read and write in order to follow written instructions and complete required paperwork. 3. Must be able to relate well to all people of the community regardless of color, national origin, religion, sex, pregnancy, age, marital status, veteran status, sexual orientation, disability or socio-economic level. 4.
Must understand, uphold, and continue working toward accomplishing the mission, strategic goals and performance measures of the agency. Supervisory Responsibility: This position has no direct supervisory responsibilities. Work Environment: 1. Requires working indoors in a temperature-controlled environment with some exposure to copier and printer chemicals/fumes.
2. The work environment may include some exposure to aerosol transmissible diseases (ATD). As part of an employee health and safety program, personal protective equipment is provided as needed. Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
Physical 1. Position requires intermittent sitting, standing, walking, twisting, and bending. 2. Must be able to lift up to 40 pounds.
3. Simple grasping and hand manipulation required as well as reaching above and below shoulder level. Travel: Some travel may be required to attend meetings, conferences and trainings. License & Certifications: Must have dependable, insured transportation, and a valid California Driver License (mileage to be reimbursed) and acceptable driving record.
A DMV printout and proof of insurance will be required. Additional Eligibility Qualifications: 1. Must be at least 18 years of age by date of hire. 2.
Must comply with the CAPSLO COVID-19 Vaccination Policy, either being fully vaccinated or have an approved exemption. 3. Based on program, a current TB skin test/Chest X-Ray may be required. 4.
Employment is contingent upon meeting all applicable job requirements and background requirements. A criminal history clearance is required prior to employment. 5. Upon hire, agency staff will check the Megan’s Law (or the current sexual predator website) to ensure each staff being hired is clear for employment.

CAPSLO pays the full amount (100%) of the premiums to cover full-time employees, and half (50%) of the premiums to cover eligible part-time employees. Vacation/Sick Leave CAPSLO awards 80-160 hours of vacation annually with a maximum accrual of 320 hours, depending on years of service.
Paid Sick time is accrued at 8 hours per month (based on weekly hours) with a maximum accrual of 280 hours.
