ABOUT THE ORGANIZATION
Providence Support Services, Inc. has been providing services to adults who have intellectual, developmental and/or physical disabilities on Medicaid funded waiver programs since November 2006. PSSI employs staff to work in the Community Living Services Supported Living program and Community Integration Services Customized Community Supports program in Bernalillo county. Community integration & interaction are key parts of the programs that are strongly promoted.

PSSI's employees advocate for the individuals they support to raise community awareness & lessen stereotyping of people with disabilities. Employees working in all programs will assist individuals with different levels of abilities in developing the life skills necessary to live their lives to the fullest and make the most out of everyday. Employees assist each individual with daily living skills, medication, cooking, cleaning, grooming/hygiene, medical appointments, health, safety, community activities, transportation, etc.

Experience is not required to work at PSSI in entry level positions. We hire many people that have never worked in the field before. Employees receive training to support adults who have intellectual, developmental and/or physical disabilities before working with individuals and throughout the course of their employment.
Registered Nurse (RN) Job Description:
Summary of Duties:

The Registered Nurse (RN) position is safety sensitive, salaried and exempt. The Registered Nurse (RN) is responsible for supporting the nurses’ professional responsibilities regarding documentation and reports and following procedures for healthcare coordination for program participants as defined in the New Mexico Board of Nursing Rules/Nurse Practice Act, DOH/DDSD DD Waiver Standards, and state and PSSI company standards, policies and procedures. Some of the required documentation for the RN to complete are e-CHATs and MAATs, ARST, Healthcare Crisis Plans, Medical Emergency Response Plans, Monthly Nursing Assessments, semi-annual reports, Missed Medication Protocols, staff training and documentation for HCP/MERPs, Therap, iTherapy, and Quickmar database entries, etc. The RN must assure participants are getting to their appointments, receiving the yearly checkups they need, that their health is monitored and health care needs are met, must attend program participant appointments, update medical databases and respond to on call including but not limited to: PRN medication calls, health related and emergency medical calls. The RN is an essential employee and also responsible for the additional duties as listed below and/or assigned:
Primary Responsibilities/Essential Job Duties and Functions:
Agency Policies and Procedures:
  • Abides by the Employee Conduct Policy, Code of Ethics, HIPAA confidentiality, and all other policies;
  • Exemplifies agency Mission;
  • Works schedule as assigned;
  • Maintains good attendance and punctuality;
  • Assists in assuring the health and safety of the program participants;
Medication Supervision:
  • Ensures medications are ordered as required;
  • Assures facility supervisors are storing medications securely and appropriately in locked med closets and boxes-example: per program participant in accordance with AWMD rules;
  • Coordinates Cycle Fill and receives daily deliveries of medications to the PSSI office from the pharmacy, checks it in and distributes it to the facilities;
  • Completes reviews of MARs and medications and assures facility supervisors and direct care staff are keeping them orderly and current;
  • Reviews Pharmacy information and assures facility supervisors are keeping them orderly, and current;
  • Ensures that medication labels coincide with the prescription and MAR and also ensuring all information required by NM DOH DDSD and/or NM DHI/QMB is on the MAR in Quickmar and mediation label;
  • Reviews, signs and ensures the mars (medication administration records) are accurate and updated as needed;
  • Receives calls and evaluates program participant use of PRN medications for approval or non-approval;
  • Assures Supported Living and Customized Community Supports programs are complying with the New Mexico Board of Pharmacy, New Mexico Board of Nursing rules and DDSD policies for medication.
  • Calls the pharmacy to ensure adequate amount of medication on hand- may must take charge of medication refills to avoid medication omissions.
  • Provides assistance with and support for medication delivery in compliance with the DDSD AWMD training
General/Documentation/Reports to be Completed:
  • Train, by video, and utilize a competency to record each direct care staff’s ability to safely follow and implement the Healthcare and Medical Emergency plans. The nurse shall also document training regarding the MERPs and HCPs delivered to agency staff and other team members, indicating knowledge competency determination for each trainee;
  • Completion of yearly Health Assessments: CHAT with Health Passport, ARST, and Medication Administration Assessment/Tool (MAAT), AT Inventory and Logs, etc. with updating as necessary;
  • Completion of the Healthcare Plans and MERPs with updating as necessary, Missed Medication Protocols with updating as necessary, and Monthly Nursing Assessments;
  • Updating, scanning and maintaining Therap and medical documentation, appointment tracking and MAR/prescription tracking databases;
  • Required to document in all company assigned electronic documentation modules including but not limited to Itherapy, Therap, Quickmar, etc.
  • Completing other medical related documentation as assigned or as the need arises;
  • Complete a nursing/healthcare semi-annual and annual report that indicates current health status of each program participant, progress to date on health related ISP desired outcomes and action plans as well as progress toward goals in any healthcare plan;
  • Completes and maintains call log of all PRN Medication calls, assuring PRN’s are documented properly on the MAR;
  • Ensures medical appointments of program participant appointments are attended and completed, annual screenings are completed, follow up appointments are made and attended, including notifying team members of medication changes or other pertinent information from appointments;
  • Ensures receipt of doctor’s orders and pharmacy receipts assuring the pharmacy receives them and the facilities have copies in their medical/day services books;
  • Updates and maintains medical related calendars, Therap, and other databases with medical and health care information; Other documentation as assigned or as the need arises;
  • Works alongside the other agency RN(s), Healthcare Coordinator, Healthcare Assistant and Program Director, and oversees LPNs, CNAs and CMAs;
  • Manages care transitions along with other members of PSSI healthcare and programming team, including changes in acuity levels, hospital discharge, other transitions related to an Out of Home Placement (OOHP), or changes between providers;
  • Assists with coordinating with the MCO Care Coordination staff to assure continuity and access to healthcare services as well as availability and access to medications, medical equipment and healthcare supplies;

Program Participant Needs:
  • Knows program participant’s plans and medications;
  • Maintains on-call status for staff questions regarding PRN medications, non-emergency and emergency medical situations; including attending to program participants at the ER, hospital, urgent care, mental health centers, etc. and face to face visits in the residential home after hours and on the weekends and face to face visits during customized community supports hours including during non-traditional hours;
  • Has access to program participant Permanent, Program and Medical records, etc.;
  • Will take vitals and administer appropriate nursing care according to the Nurse Practice Act and carrying out doctor’s orders;
  • Collaborate and communicate with a DD Waiver provider nurse from another agency when a program participant is in CCS-Group services that is not also in PSSI’s supported living program, acting in accordance with the NM Nurse Practice Act, to immediately communicate any health issues or concerns;
Monitoring Staff:
  • Report suspected program participant neglect, exploitation or abuse;
  • Review MARs, pharmacy book, residential working books and case file at least 1 time monthly;
  • Participates in interviewing/hiring/termination process;
  • Participates in review/raise process;
  • The RN must provide face-to-face supervision of LPNs, CNAs, CMAs and direct support employees who have been delegated nursing tasks as required by the NM Nurse Practice Act and the DD Waiver service Standards;
  • LPNs, CNAs, and CMAs on duty or on-call must have access to an RN by phone in case consultation is required;
  • Provides training and oversight to staff to consistently implement HCPs, CARMPs, MERPs, and doctors’ orders for persons with health needs during CCS and Supported Living;
Program Participant Appointments:
  • Coordinates/schedules, carries out/attends program participant appointments, including but not limited to emergency room visits, hospital visits/stays including discharge meetings and planning;
  • Attends IDT/ISP meetings as necessary;
  • Follows up with House Leads to ensure they know all pertinent information regarding medications and medical information from appointments; i.e., medical, dental, psychiatric, etc.;
  • Enters appointments on the medical calendar, sends invites to team members;
Miscellaneous:
  • Attends office personnel, transition and CARMP meetings;
  • Attends DOH DDSD required trainings, therapy trainings (including transferring and ambulating) and other meetings as scheduled.
  • Participating member of the Human Rights Committee as assigned;
  • Required to be aware of and follow the NM Nurse Practice Act, NM Board of Pharmacy standards and regulations and the NM DD Waiver Service Standards;
  • Has continued communication with Healthcare Coordinator and Assistant, Lead Supervisors, Program Director and RN(s) regarding important medical information, program participant appointments, medications, trainings, meetings dates and times and any rescheduled dates and times;
  • Keeps his/her office organized and clean, keeping documents and books within office organized and filed as well;
  • Maintains current First Aid and CPR certification;
  • Other duties as assigned;
On-Call:
  • A nurse is required to be available to direct care employees and be able to respond within 15 minutes by phone and within 60 minutes in-person to assess the person if deemed necessary per prudent nursing judgement.
  • Additionally, a Lead Supervisor, the Healthcare Coordinator, Executive Director or Program Director may direct a nurse (RN) to complete a face to face assessment to determine the appropriate action.
  • Must be available to receive calls during shift, after hours, and on weekends for emergency and non-emergency situations and be available to go to PSSI facilities or into the community for health-related issues and assess program participants.
  • Must be able to respond to emergency room, urgent care and hospital visits for participants (including staying with the participant) during shift, after hours, and on weekends for emergency and non-emergency situations.
  • Must return calls by phone within 15 minutes from direct care employees, emergency calls within 15 minutes and all other non-emergency PSSI calls within 24 hours.
  • On-call RNs are required to document the calls they receive, the actions they have taken or directed to document in required forms and communicate with their agency peers as well as to follow up as needed on the person’s status.

Crisis Intervention Technique:
  • Follow all behavior support plans for program participants;
  • Utilize the Company’s approved crisis intervention technique if approved in the behavior support plan;
  • Employees must a step back when a program participant has a challenging event and/or behavior. Restraint is used as a last resort. Utilize restraint only if the program participant is trying to physically hurt themselves, someone else or put themselves in immediate danger.
Safety Sensitive Position:
The RN position is a safety sensitive position where the employee holding this position has the responsibility for their own health and safety, as well as, the health and safety of the program participants, their co-workers, community members, team members and stakeholders. This position has many safety sensitive duties including but not limited to working directly with community stakeholders, supervision of employees when related to healthcare/medical, coordinating and receiving medications from pharmacy and pharmacy/medication related documents, assessing program participant’s health care needs, attending appointments for program participants, and operating a motor vehicle regularly for the company.
Working Conditions/Physical Exertion/Dexterity:
  • Works primarily in the administrative office, within residential homes, attending appointments, attends ER/Hospital events with participants in the community, and dealing with pharmacy;
  • Completes residential facility visits at minimum 1 time per month;
  • Completes program participant face to face assessments through visits to the CCS or Supported Living programs, mornings, evening or afternoons;
  • Must complete monthly nursing assessments at minimum 1x per month checking for skin breakdown, checking and trimming fingernails and toenails, taking vitals, etc.;
  • Transports program participants to appointments;
  • Schedule requires flexibility and ability to respond to on-call. May be requested to work other shifts on occasion;
  • Able to work in a residential facility or community as needed and flexible as needed to come in early or stay late.
  • Position may be changed as deemed necessary.
  • Must be able to physically perform all job duties. Moderate physical exertion required, including but not limited to bending, sitting, standing, squatting, kneeling, running, walking, climbing stairs, lifting, transferring, participating in sport activities, etc. Must be able to stand and walk continuously;
  • Must be able to lift 25 pounds above the shoulders, 40 lbs. or more repeatedly, and support 70 to 100 pounds;Must be able to safely transfer program participants from chair to car/van, assist with toileting, and other duties related to transfer/ambulation; including rising from and sitting down and laying down positions.
  • Able to move/respond quickly to many difficult situations and handle stressful situations calmly.
  • Able to relate positively to the program participants and their teams.
  • Able to work directly with program participants including but not limited to challenging behaviors, physical restraint, medical care, personal care, grooming, hygiene, assisting with medication, lifting and transferring.
  • Able to work alone with program participants who are or can become verbally and/or physically aggressive on a daily basis and support them through crisis events.
  • Able to hear and withstand repetitive noises, sounds, phrases, yelling, screaming, etc.
  • Able to assist, support, lift and transfer individuals who utilize walkers, wheelchairs and gait belts;
  • Able to perform the essential functions of the job with reasonable accommodation.
  • Able to work independently without constant supervision.
  • Able to supervise staff, train staff and complete disciplinary process with staff.
  • Must have good verbal and written communication skills, good organizational skills, and is a team player;
  • Mandatory influenza vaccine annually;
Education/Training/Experience/Licensor Requirements:
  • Current RN licensure in compliance with the NM Nurse Practice Act throughout the course of employment
  • Minimum of 1 year of experience in this field/related field preferred
  • 21 years of age or older (required by company commercial insurance)
  • Must be able to use a computer, fax machine, copy machine, multi-line phone system and other electronic equipment proficiently, navigate the world wide web/internet, type, create and maintain databases and spreadsheets.
  • Must have a clear driving record at hire and throughout the course of employment - with less than 2 moving violations within the last 2 years and no DUI/DWI’s within the last 5 years;
  • Must be able to pass a pre-hire drug/alcohol screen and continue to pass screens throughout the course of employment;
  • Must have a clear criminal background at hire and throughout the course of employment, including but not limited to COR/EAR check, NM Courts, sex offenders, receiving a clearance letter through DOH CCHSP,etc.;
  • Must have a current valid NM driver’s license at hire and throughout the course of employment;
  • Must comply with all training requirements, including but not limited to general orientation, DOH state mandated and therapy training;
  • Must have and maintain current personal liability auto insurance in your own name or provide proof your name is on the policy at all times while employed;
  • Must have your own reliable vehicle throughout the course of employment;
  • Must be able to operate company vehicles, drive for the Company and transport program participants in your own vehicle and company vehicles throughout the course of employment;
